A First-Rate Facility and Staff: We are extremely satisfied with the care and rehabilitation services our 87 yr old received here. Michelle, our case manager, provided outstanding clarity, helpfulness, responsiveness, and compassion in guiding us through the rehab process. When my turn comes for professional rehab services, I will not hesitate to trust myself to Centre's care.
Centre Ave. failed my dad in every possible way. My dad passed away in August 2024. He fell ill just two days before being transported to the ER. On the day my dad started feeling sick, my brothers, mom, and I visited him and asked staff to check on him because he was complaining of a sore throat and having trouble breathing. Centre Ave. lost his inhaler, which exacerbated his condition. Despite our requests, no one came to assess his situation. My mom had previously also expressed her concerns about my dad potentially falling out of bed and had asked multiple times for staff to put up barriers and padding on the floor for his safety. Unfortunately, they did not take action. The next day, my mom once again asked for assistance as my dad was still unwell. She waited and waited for help, finally, they informed her that she would need to take him to the doctor herself. At that point, my dad couldn't walk, hence the reason he was in rehabilitation. This was an unreasonable and inappropriate option. My mom was understandably upset and made several attempts to get someone to help her. She left later that day, and the next morning, she received a call from Centre Ave. stating that my dad had fallen out of bed during the night and was not feeling well. They waited until 9:30 AM to inform her of this!!!! My mom rushed to Centre to find my dad propped in a chair, clearly in distress. He was struggling to breathe, and she insisted he go to the ER. It took a couple of hours for that to happen. By the time the ambulance arrived at the ER, my dad was in respiratory failure, diagnosed with double pneumonia and dangerously low oxygen levels. The ER doctor informed us that the only option was to place him on a ventilator or he would die within minutes. This was because the pneumonia had progressed and my dads condition was dire. We had to make a rapid decision, opting for the ventilator. My dad remained on the vent for nine days with little progress. We were advised that if he survived, he would never be the same and would lack a good quality of life. Eventually, we made the heartbreaking decision to take him off the ventilator, and he passed away. The employees at Centre Avenue never showed any urgency or genuine concern for my dad, and they displayed no remorse for their shortcomings. I reported these issues to the Larimer County Health Department, which conducted an investigation and concluded that Centre Avenue was not at fault. They seemed to find it acceptable for a patient to fall from his bed without notifying the family until the 9:30am the next morning. I believe my dad had fallen sometime during the night and they found him in the morning. I believe he was on the floor most of the night. They called my mom late because they knew they were negligent. Moreover, the investigation found it appropriate when there was no medical attention given to a patient who, under their care, became severely ill due to negligence. The staff at Centre Ave. did everything wrong. EVERYTHING! They allowed my dad to suffer a fall, lost his inhaler, and failed to provide timely medical attention when it was requested. As a result of their negligence, my dad's condition deteriorated rapidly. By the time any action was taken, it was too late. My dad would be alive today had they done their jobs and provided the medical care my dad so desperately needed. If sharing my family's experience can save another family from enduring what we went through, I will feel satisfied. I trusted them to care for my dad. I will forever regret the decision to allow Centre to care for my dad. DO NOT allow Centre Avenue Rehab to care for your loved ones. Sincerely, A daughter who misses her daddy:( P.S. For the most part, the staff is great. However, the nurses are understaffed and overworked. They are great; they just can't handle everything. So visit your loved one often and ask a lot of questions. The people at the front desk are very friendly and greet my step mother by name. The PT staff made significant progress with my dad when The Centre at Rock Creek hardly made any progress in the 2 1/2 months that he was there. All in all, this is one of the better run Columbine facilities.
Typically I went to see my dad in the afternoon. Saturday July 22 I went in at 10:00 a.m. I found my dad without any clothing and flat on his back with no water or oxygen within reach. He was dying from congestive heart failure. Everybody knows you do not lay a person flat that has congestive heart failure. It was a complete nightmare and I removed my dad 2 days later. A pathways hospice nurse (Triva) was incredibly unprofessional and talked me out of removing him immediately saying he "has hours to live" which was not true. I do have photos of this but obviously I will not share them with the public. This place is definitely a place not to take your loved one.
Although most staff seems friendly, they didn't seem interested in admitting my LO. They didn't seem concerned with her needs and requirements. Once I picked up my LO to bring her home, her cath bag was so full it was about to explode, she was left to sit in feces and they ripped her toenail off and nobody knew how. It was a heartbreaking experience that I refuse to ever let happen again. I do not recommend this place. I did give 2 stars because a couple of the nurses seemed to make a heartfelt impact on my LO.
