My mother was transferred to Heritage Lakeside for 1 month after suffering a stroke. We were forced to send her as it had immediate openings. This facility is an absolute dump, filthy dirty, and the rating of F proves this. You can tell that the owner of this FOR PROFIT facility has no interest in making improvements. The staff and therapy team were great, but can tell that everyone is run down, over worked, and short staffed. Patients receive only 1 shower per week (if that) and found my mom put in dirty clothing numerous times. We were so thankful to finally get her transferred to a new facility. We can now sleep at night knowing that she’s in a safe place. Please look up state violations before considering this facility.

Angie, NHA and her team do an excellent job of meeting both the clinical and personal needs of the residents they serve! I’ve been visiting this nursing home for the past three years. I have seen such great improvement in all areas. This facility is on its way to being a 5 Star nursing home. There is such a positive vibe I feel when I visit this building from both the residents and the staff who serve them. I highly recommend them for short-term, rehab or long term care needs! Staff is well trained, management is amazing, residents are well cared for and have all of their needs met. Reasonable business hours and amazing support for staff residents, and family.

My Mom was transferred from Marshfield Hospital to Lakeside Heritage in 2023. Very slow submitting bills to insurance companies. Wants the families to pay for monthly bills that Heritage Lakeside submitted late to the insurance company. My parent's insurance company said, "families are not responsible for late bills submitted to insurance company." Transferred my mom in 2024 to an assisted living where is she getting better care and is happier where she is now living. She loves the chief's food. Have no more problems insurance paying monthly living bills for my mother. Heritage Lakeside replaced the floors, painted the rooms and painted the doors grays with cheap sloppy company while my mom was a resident there for 1 year and 5 months.

My father was here. I was grateful for an opening. He was treated with dignity and respect. He had a partial hip replacement. He was in his 90's. He had rehabilitation and returned home. I felt the staff was very sweet and kind. I was able to visit him quite often. They arranged transportation for him for his appointments. They receive 5 stars from me!
