Home Health Hiring LVNs

We are seeking a Licensed Vocational Nurse - Home Health to join our team! You will be responsible for the assessment, diagnosis, and treatment of assigned patients in a home health setup. Please note that our rates are per visit/case basis. Responsibilities: • Providing direct patient care under the supervision of RN. (In-Home Visits) • Administer the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. • Providing accurate and timely documentation consistent with the plan of care • Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care. • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor. • Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique. • Participates in personal and professional growth and development. • Performs other duties as assigned by the registered nurse. • Provide advice and emotional support to patients and their family members Qualifications: • Previous experience in nursing or home health is preferred • Ability to build understanding with patients • Strong problem solving and critical thinking skills • Ability to thrive in a fast-paced environment Requirements: • Professional License • Updated CV/Resume with Experience • Driver's License • Auto Insurance • Active CPR or BLS Certification • Health Certification or Recent Physical Examination/Fit to Work note • Valid TB/PPD Test or Valid (3 years) Chest X-ray • Covid Vaccine record ***including booster • Influenza Vaccine (or Declination) • Reference Check
