Licensed Vocational Nurse (LVN) Job Description

Purpose: The Staff Nurse provides nursing services to residents in the assigned work area in compliance with current applicable federal, state, local and facility standards, guidelines and regulations and as may be directed by the Administrator to assure that the highest degree of quality health care of our residents is maintained at all times. Reports To: Administrator Primary Job Duties: (* denotes essential functions) *1. Directs and renders care to the residents within an assigned unit as a licensed staff member. Fill needs of schedule and assignment as needed. *2. Provides leadership by working cooperatively with nursing assistants and caregivers. *3. Receives doctors' orders, charts, assists physicians, supervises other nursing staff. *4. Reviews medication orders and medication sheets for completeness and accuracy of information, accuracy in the transcription of physician orders, and adherence to stop order policies. *5. Provide direct resident care as directed and when needed. (i.e., assist with lifting, transferring, turning residents). *6. Makes daily visits to resident's room to observe and evaluate physical and emotional status/needs of the residents during shift. Notifies Assisted Living Coordinator of any change in resident condition. *7. Receives and follows up on resident report from nurse on previous shift. *8. Administers prescribed medication and treatments as ordered by the physician. Charts same and notes all effects. Note: LVN's can only administer treatments they are authorized to administer. *9. Notifies physician and family members or responsible party of any change in the resident's condition according to facility procedure. *10. Completes incident reports promptly on residents and on employees when necessary. *11. Assists in the job performance evaluation of nursing staff which are assigned in work area. Assists in the counseling of staff in areas of job performance conduct and quality of care delivered. *12. Converses with residents and their families regarding problems and possible solutions, progress and plan or care and conveys a genuine sense of concern. *13. Performs all duties in a safe manner and insures that staff follows infection control procedures and universal precautions in accordance with facility procedures. *14. Improves professional knowledge through professional reading, in-service education and other avenues. Attends in-service education sessions on a regular basis. *15. Maintains confidentiality of all pertinent care information to assure protection of resident rights. *17. Participates in Overall Plan of care for each resident. Assists in completion of Service plans and level of care assessments. *18. Assists in orientation and training of newly assigned personnel in job duties on unit as well as supervises and instructs other nursing personnel. *20. Answers telephone inquiries in a professional manner. *21. Orients new residents and their family members to functions of the Department. Performs other duties as necessary and appropriate or as directed by the Administrator to fulfill resident needs. Educational and Experience Requirements: The Licensed Nurse must be licensed as a vocational nurse by the State of California, and be a graduate of an accredited school of nursing. License must be kept ACTIVE and in good standing with the State of California. Other Requirements: Must deal tactfully with staff, residents, family members, visitors, government staff and the general public. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care. Must possess supervisory ability, good communication skill, and the willingness to work harmoniously with professional and non-professional staff.
