Hiring Full-time/Per Diem RN positions for Home Health

Coverage needed: Inland Empire, Temecula Area, San Bernardino Area, Riverside Area Job Summary: Evaluates and provides family-oriented services to home health patient with neurological, musculoskeletal, orthopedic, and cardio-pulmonary impairments; fine motor, sensory motor, and developmental delays; as well as traumatic injuries and swallowing disorders, this includes patient and caregiver education; discharge planning; and consultation with other professionals, clinics or agencies. Essential Functions: Performs initial and ongoing comprehensive assessment of the client’s needs, including Outcome and Assessment Information Set (OASIS) assessment at appropriate time points. Initiates the Plan of Care and revises as necessary. Provides services and/or treatment requiring substantial and specialized nursing skill. Counsels the client and family in meeting nursing and related needs. Initiates appropriate preventive and rehabilitative nursing procedures. Prepares clinical progress notes, evaluates outcomes of care, and plans for discharge from services. Coordinates services and referral of other services as needed. Maintains communication with physicians, family members and other agency personnel to adequately plan for and provide the patient’s needs. Supervises LVNs, CHHAs and paraprofessionals providing services. Attends and participates in continuing education programs. Adheres to all company policies and procedures. Demonstrates flexible and efficient time management techniques. Prioritizes workload so as to meet productivity standards and applicable deadlines. Maintains a positive and respectful attitude and demeanor. Maintains strict confidentiality with regard to protected and sensitive information. Knowledge, Skill, and Experience: One (1) year of experience in Home Health preferred. Minimum of one (1) year in the acute care setting. Currently licensed by the California Board of Registered Nursing. Must be able to maintain strict confidentiality with regard to protected and sensitive information. Must be able to solve problems independently and confidently by applying analytical and logical thinking.
